This practice is piloting a system that allows you to view your medical record using a personal computer (PC) and the internet.
The system is a website that enables you to view your medical record over the internet from a PC. It allows you to easily and quickly view the electronic medical information held about you by your GP Practice
The document (link below) explains what the system is and how you can use it. Before you can begin using the Online Electronic Health Record (EHR) viewing system we require you to register and consent to use the system in the GP practice.
*If you are already registered for EMIS Access and booking appointments online simply read the document fully and complete the questionnare and consent form then return to the practice.
*If you are not already registered for EMIS Access you will be required to bring in 2 independant forms of I.D. (one must be photographic i.e. passport or driving licence and the second should preferebly be a utility bill confirming your address) at the same time you bring in your consent form and questionnaire.
